FIVES: Facility Information, Vacancy and Evacuation System

Texas Department of Aging and Disability Services


The department has created FIVES in order to help providers of LTC services help one another during emergencies such as hurricanes and other disasters that may require mass evacuations. Please print these instructions for future reference.

To update your facility information. vacancy and evacuation status

Record Your Facility's Number of Vacancies, Evacuees Admitted and Evacuation Status

Instructions:

Step 1: Click the link above. When you login the very first time, select your facility from the list. The password will be the facility id listed next to the facility in the list.

Once you are in the system, take a moment to choose your own password. Click the Change Login Information icon in the top toolbar, and choose your own password. Be sure that you save this password in a secure place where you can find it if you forget it.

Step 2: If you are evacuating your facility, click the Report Evacuation icon in the toolbar or click the page link that says, "To change the evacuation status of your facility, click here" and then see Step 6 below.

Step 3: Otherwise, update your vacancy information. Select the number of vacancies from the dropdown box.

Step 4: If you have admitted evacuees from other facilities during the present crisis, please enter the total number of evacuees that you have in your facility at this time.

Step 5: Click the Update button. That's all you need to do. Ignore the remaining steps, and close your internet browser.

Step 6: If you are evacuating your facility, use the text box to enter information explaining where you are sending your residents. Do not enter personal information that would violate HIPAA privacy requirements. For instance, you might enter a note such as

14 residents sent by bus to Concordia ALF in San Antonio.
23 residents sent by bus to East Village ALF in Dallas.
The facility was secured and closed on 7/3/2006 at 2PM.

Step 7: Click the Submit button. Note that you cannot mark your facility as evacuated without entering a note. Once that is done, close your internet browser.

To find facilities that can accept your residents

Find LTC Facilities with Vacancies

Instructions:

Step 1: Click the link above.

Step 2: At the top of the report window, set the Report Type dropdown box to indicate Vacancies.

Step 3: Next, set the Facility Type dropdown box to indicate your provider group. For example, if yours is an Assisted Living Facility, select Assisted Living Facilities.

Step 4: Use the next three dropdown boxes to choose how you want the report to be sorted. If you want only two levels of sorting, say by City and number of Vacancies, set the third sorting control to Vacancies too. Once all the dropdown boxes have been set, click the Go button to sort the report.

Step 5: Use the information in the report (number of vacancies, location, etc.) to find facilities that may be able to accept your residents. Phone numbers are listed so that you can contact facilities in order to determine whether they can help by accepting your evacuees.

Step 6: To generate a map from your facility to the receiving facility, click on the Map link in the same row that corresponds to the facility that has agreed to accept your residents. Note that each map link is different; you cannot get directions to a specific facility unless you use the map link for that particular facility.

Step 7: In the mapping window, use the link Directions To near the top of the map to get directions from your facility to the destination facility.


The Department of Aging and Disability Services cannot warrant that information voluntarily submitted by LTC facilities is current or complete. The value of FIVES depends entirely on the participation of LTC providers.
